Terms and Conditions and Privacy Statement

General Rules and Definitions
This website serves as the application platform for The MAC Fellowship Program (“Fellowship”) administered by The Robert B. McMillen Foundation (“Foundation”). These terms and conditions apply only your application for the Fellowship and do not apply to any other interactions with the Foundation.
1.2. The Foundation may change, add, or remove portions of these Terms at any time in Foundation’s sole discretion. Notice of changes to these Terms will be posted on the Foundation website. Additional terms and conditions may apply, including those described at https://www.surveymonkey.com/mp/legal/fluidreview-smapply-tos/. Before you submit an application for the Fellowship, please confirm that you meet the most recent eligibility requirements posted on the Foundation’s website. Unless otherwise stated in the eligibility requirements, you may only submit one application for each Fellowship. All applications must be received prior to the due date stated in the eligibility requirements.
You agree that all information submitted on a Fellowship application is true, accurate, current and complete at the time submitted. The Foundation may reject any application which contains information that the Foundation suspects may be untrue, inaccurate, incomplete, or is not current. You agree that the artwork you have submitted for consideration of the Fellowship is your original work. No Fellowship application materials will be returned, under any circumstances.
To complete the Fellowship application, you will be asked to provide personal information. You further agree to authorize the release of any information reasonably requested by the Foundation to confirm and/or verify this application. We will not share your personal information with third parties who are not providing services on our behalf. We will use your personal information to analyze your application, share your application with our selection committee, and we may use it for other internal purposes, including contacting you for Foundation purposes and analyzing our applicant pool. You acknowledge and agree that Foundation may share your personal information to non-Foundation employees who may be participating in Fellowship determinations or supporting the Fellowship application platform or process. Foundation also has the right to provide account, content or use records, and related information in response to legal responsibility, lawful process, orders, subpoenas, or warrants, and to protect our rights. Foundation maintains industry standard safeguards to secure the privacy of the personal information you submit. Foundation will keep your personal information only for so long as there is a reasonable purpose for retaining your personal information.
By choosing to submit your application via the Foundation online application system, you agree that Foundation may contact you directly regarding your application and/or the funding program by using the email address and/or phone number that you have provided in the registration or application.
Termination and Suspension
You may terminate your account at any time by sending email to firstname.lastname@example.org. Foundation may terminate or suspend your access to all or part of this platform for any reason, including breach of this agreement. If at any time Foundation is unable to operate this Fellowship application platform or the application process as planned (for example due to financial or administrative difficulties, fraud or technical failure such as a computer virus/bug or other form of unauthorized intervention that corrupts the administration, security, fairness, integrity or proper conduct of any campaign), Foundation reserves the right, in its sole discretion, to suspend, modify or terminate the application process. In the event of any such termination prior to the end of an application time period, notice of such termination will be posted on the Foundation website.
